About this service

What you can expect

Existing MedVibe patients may request eligible return-to-work, school, and insurance forms when they completed a MedVibe consultation no more than 15 calendar days ago and the chart contains the required information. Income-replacement forms, disability forms, and other complex administrative attestations are not accepted. If a new assessment is required, the separate $150 tax-exempt consultation fee and the taxable $50 form fee both apply.

How this service works

This service starts with a non-clinical support inquiry. MedVibe then provides the secure channel for the form and any required information. Do not use Fast Booking for the $50 form fee.

Clinician discretion. All prescriptions, notes, requisitions, and treatment plans are provided only when clinically appropriate and at the clinician's discretion.

Nurse practitioner review
A nurse practitioner reviews and signs eligible forms, normally within 24–48 hours after the form, chart information, and payment are complete.
